SYMPTOM:

Black screen with Windows NT Server 4.0 and the IBM S3 video driver.

Note: If system encounters a blue screen problem, the S3 video driver hangs before the text based blue screen is displayed.

       (Note: These are affected with or without any service pack applied.)
