PSF/AIX:Problems with S/370 Channel Emulator

I'm attempting to attach a 3900-001 to an RS/6000 running AIX V4.1.4.0
with PSF for AIX V2.1.0. I've installed the latest maintenance for PSF
including U445267; U444135; U443128 and U445497. Listing the levels I
get psf.base.02.01.0000.0000.U446800
psf.man.02.01.0000.0000.U443128
psf.pcl.02.01.0000.0000.U445267
psf.ps.02.01.0000.0000.U444135
psf.sap.02.01.0000.0000.U446100
The adapter card seems to be installed fine;
lsdev -C | grep chna0
returns the response
chna0 Available 00-04 IBM S/370 Channel Emulator/A Adapter
I then proceed with the installation of the driver as described in
the Print Administration manual and the response is that
afp0 has been successfully installed.
However when I give the command
lsdev -C | grep afp
the response is:
afp0 Defined 00-04-00 IBM S/370 Channel Emulator /A Printer Driver
i.e., the driver DID NOT install successfully.
When I continue to smitty - Add a Printer
S/370 Channel Emulator/A Adapter
there are no slots available under slot number on the menu so
there is no way to define the printer.
What's going wrong????

It is the loading of the device driver onto the card that will change
the state from Defined to Available, so the device driver has not been
successfully loaded, despite what smit might indicate.
Have you tried running the "sense" command as described in the Print
Administration manual? If so, what are the results?
One thing to check is that these S/370 Channel Emulator/A adapters can
only be installed in BUS0 of a multi-bus system; please ensure that
you have the card installed in BUS0. (Most RS/6000s only have a single
bus, but in the event that you have a multi-bus system, you must install
the adapter in BUS0.)
Also, did you follow the instructions that should be included with the
PTFs which state that you need to re-IML the RS/6000 after installing
the PTF that has the device driver for the channel card? If this is
the first psf.base maintenance you've installed in a while, you may to
re-IML the system so the device driver gets added properly, and then
go through the steps to load the device driver onto the card.
Does the customer have more than one SCSI adapter in the RS/6000?
If so, there is a known problem with adding the S/370 Channel Emulator/A
adapter to that system; fortunately there is a workaround that the AIX
Support Center can assist with. Basically it involves using the rmdev
command to remove some of the SCSI adapters (be careful not to remove
the adapter supporting your hard disk with the rootvg volume group¢),
then adding the channel adapter, and then readding the SCSI adapters.
If you are still unable to make the card "available", you should
open a formal problem through your support channels. I hope this helps.
