Stale PPs and syncvg does not help


Question:

I have two model 370's running AIX 3.2.5.  I have 2 1GB drives and 2 
2GB drives.  These drives are in a volume group called usr2.  I have 
the logical volumes in this volume group mirrored.  

I just recently noticed stale pp's on hdisk1 and syncvg does not help.  
When I issue the command "lspv hdisk1" the PV State comes up as "missing"
and all of the PPs are showing up stale.  "lsdev -Cc disk" does show 
hdisk1 as available.  I have had the hardware checked and everything 
checked out okay.  What is the problem and how can I correct this
situation?

Response:

"lsvg -p usr2" showed all disks as active except hdisk1 which was 
"missing".

"lspv hdisk1" showed a PVID of 000026328dafe6db.  "/usr/sbin/hdf 
/dev/hdisk1 80 20" showed the same PVID.

"lqueryvg -p hdisk1 -At" showed no Logical Volumes and only one PVID in
the volume group.

However, "lqueryvg -p hdisk2 -At" showed three logical volumes and 4 
PVIDs including the PVID for hdisk1.

It appears that hdisk1 still belongs to the usr2 volume group, but for
some reason, hdisk1 has a VGDA for a new volume group on it that only
includes hdisk1.  

To fix this situation, first, verified that hdisk1 only contains
mirrored copies with "lquerypv -p`getlvodm -p hdisk1` -N hdisk2 -d".  
Then use the following steps to remove the mirrors, remove hdisk1 from 
usr2, re-add hdisk1 to usr2, and then re-add the mirrors:

  rmlvcopy lvusr2 1 hdisk1
  rmlvcopy usr2log 1 hdisk1
  reducevg usr2 hdisk1
  extendvg usr2 hdisk1
  mklvcopy usr2log 1 hdisk1
  syncvg -l usr2log
  mklvcopy -ex lvusr2 2 hdisk1 hdisk4
  syncvg -l lvusr2
