0301-155 bosboot: mkfs failed for /var/tmp/blv/disk.fs.

Env:
  AIX 3.2.5
  RISC 58H
  sysback 3.x.x.x

Desc:
  Customer was reinstalling from a sysback full system backup
  and at the very end of the backup it failed with this message:

  0506-526 cannot determine log device to use for /dev/hd5x
           Filesystem Helper:0506-526 Implementation specific 
           error code=(122)     
  0301-155 bosboot: mkfs failed for /var/tmp/blv/disk.fs.

Action:
  I had the customer choose option 1 (perform system maintenance)
  bosboot -ad /dev/hdisk0
  exit
  
Once up in normal mode, I had the customer run this command
bosboot -a -l hd5x -d /dev/hdisk1

It failed with the above error message

I had the customer check the /etc/filesystems file and it was missing
the stanza for hd5x.  I had the customer add this stanza in the file

/blv:
                      dev          = /dev/hd5x
                      vol          = "spare"
                      mount        = false
                      check        = false
                      free         = false
                      vfs          = jfs
                      log          = /dev/hd8

Now the bosboot works for hd5x
