Creating LV's/filesystems gives 0516-022 and mknod error


Env:    AIX 3.2.5, Model 59H

Desc:   Customer get the following error message when creating
        logical volume:

          0516-022 lcreatelv illegal parameter

        He was told yesterday to do synclvodm to correct problem 
        and it did.  He shutdown his machine yesterday and then
        tried to create logical volume.  It failed with same
        error message.  When he ran synclvodm, it worked again.
        Customer wants to know why he has to run this command 
        so much, this is a new machine.

        Had customer look in error report, nothing was there.
        These are all internal IBM drives.

        Summary of testing each VG:
         
        hdisk5  disk5vg  -- empty; created LV ok (4pp size)
        hdisk4  disk4vg  -- not empty; failed (4pp size)
           tried to create lv and it failed with the
           following:  0516-022 and 0516-822
        hdisk0 & hdisk1 rootvg  -- not empty (8pp size)
           tried to create lv and it failed with the
           following:  0516-022 and 0516-822
        hdisk3  disk3vg  -- not empty  (8pp size)
           tried to create lv and it failed with the
           following:  0516-022 and 0516-822 
        hdisk2  disk2vg -- not empty; created lv ok; failed before
           (8pp size)

        This looks like some ODM corruption.  He rebooted in service 
        mode copied the clean ODM over and rebooted. 

        This seemed to clean up the ODM corruption but now
        he can make a LV but when he tries to make a filesystem
        for the LV, he gets:

                mknod do not specify existing filesystem
                0516-822 mklv unable to create logical volume
                0506-902 cannot create lv

        The same error happens when creating a filesystem from scratch.

        Had customer mail in the ls -l list from /dev:

        brw-rw----   1 root     system    22,  3 Sep 28 07:51 loglv00
        brw-rw----   1 root     system    22,  3 Sep 06 10:25 loglv01

        We removed loglv01 since lslv loglv01 did not give any info.
        We then tried making the jfs and we got the same errors.

        There was still:

        crw-rw----   1 root     system    22,  3 Sep 28 07:51 rloglv00
        crw-rw----   1 root     system    22,  3 Sep 06 10:25 rloglv01

        We removed rloglv01 and all problems went away.
