0514-044 on "cfgmgr"; corrupt / and /dev/ipldevice


Env:    3.2.5, HP 4mm 4Gb tape drive

Desc:   Customer attached the drive and rebooted.  "lsdev -Cc tape" showed
        no tape drives on the system.  After ensuring that the drive was
        properly connected and verifying that the drive was set to use
        a unique SCSI address, ran "cfgmgr" and got errors:

        0514-044 Cannot write a special file
        Unable to save space on /dev/ipldevice

        The customer got the same error trying to add the tape drive
        using "mkdev" in SMIT.

Action: Checked the permissions on /dev/ipldevice; they were brw-------,
        rather than crw---------- (the correct permissions).
        Need to recreate /dev/ipldevice as a character file:

        \# bootinfo -b (confirm the boot drive)
          ( or run "lslv -m hd5" )
        \# rm /dev/ipldevice
        \# ln /dev/rhdisk\# /dev/ipldevice (from previous output)
                (Note that this is a hard link, not a soft one)

        While trying to remove /dev/ipldevice; got error:

        0653-609 Cannot remove file /dev/ipldevice.  
        There is an input or output error.

        However, the file was actually removed.
        We were able to recreate ipldevice successfully from the steps
        above.  Ran "fsck /dev/hd4" (/ was still mounted), and it
        returned a Dirty Superblock.

        Faxed steps on fixing the dirty superblock in service mode
        (superblock.recover.krn, 1-800-IBM-4FAX \#2264).

        Customer reported that after fixing /, and rebooting in normal
        mode, the tape drive was now "Available".
