0514-040 cannot initialize device in kernel.

Env: AIX 3.2.5
Desc: The system console was defined as HFT. They had
no HFT to hook to this machine, only a 3151. He
boots off of bosboot diskettes fine. He tries to
do the 'smitty chcons' command, but tty0 does not
show up, because it has not been defined on this
machine before. He tried 'add a tty' through
'smitty tty'. The mkdev command fails with the error:
0514-040 cannot initialize device in kernel.
Action: I thought that maybe this was being caused because
we were choosing the tty state to be 'available' instead
of 'defined'. I changed this, and got the same error
message. I re-created this on my system, and found a
workaround. Instead of using mkdev through smit, do
the following:
1. /etc/methods/define -l tty0 -c tty -s rs232 -t tty
-p sa0 -w s1
2. /etc/methods/cfgtty -l tty0
3. chdev -l tty0 -a login='enable' -a term='ibm3151'
Now, when you reboot the system, you will get a login
prompt on they tty, at which time you can run the
'smit console' and set the console to tty, and then
reboot the system again, and the tty will now be
the console.
