IBM IntelliStation/IBM PC - Microsoft Office 95 causes an access violation under Windows NT 4.0

Symptom:
Microsoft Office 95 under Windows NT 4.0 results in an Access Violation when the following video settings are used:
1024 x 768 and NOT 256 colors
640 x 480 and 16 million colors
Fix:
Do not use the video resolution of 640 x 480 and 16 million colors or 1024 x 768 (unless 256 colors is selected).
